If, unexpectedly, you have problems with this, you can follow the instructions below in rare cases.

01 Registration form to 06 Fee agreement: Please print out the documents, complete and sign them and bring them with you to your appointment at our practice.

09-10 Information forms: Please print out the document that applies to you and read it carefully. When you visit the practice, you will receive the original, I will explain it to you and you can ask questions. You will then certify your consent with your signature.
